Great auricular nerve: anatomy and imaging in a case of perineural tumor spread.

We present the imaging and clinical findings of a case of recurrent cutaneous squamous cell carcinoma of the face in which CT and MR imaging revealed perineural tumor spread along the great auricular nerve. The great auricular nerve is a superficial cutaneous branch of the cervical plexus, providing sensory innervation to the skin of the parotid and periauricular region. Our purpose was to fami...

Detection of perineural spread: fat suppression versus no fat suppression.

One could easily argue that, when imaging head and neck malignancy, evaluation of possible perineural spread is more important than identifying the primary tumor or finding metastatic nodes. In almost every case, the clinician has already identified the primary lesion and has made a good estimate of the margins. The location and size of the primary tumor determine the treatment of lymph nodes a...

Mandibular Canal Widening and Bell's Palsy: Sequelae of Perineural Invasion in Oral Cancer

Perineural invasion is an underrecognized route of metastatic spread along the nerve bundles within the nerve sheath into the surrounding tissues. It hinders the ability to establish local control as tumour cells can traverse along nerve tracts well beyond the extent of any local invasion rendering them inoperable and unresectable. Perineural invasion is a marker of poor prognosis. Oral submuco...

Perineural tumour spread from colon cancer, an unusual cause of trigeminal neuropathy--a case report.

Malignant trigeminal neuralgia due to perineural spread along the branches of the trigeminal nerve, is known to commonly occur secondary to squamous cell carcinomas, lymphomas and adenoid cystic carcinomas in the head and neck region. Rarely metastases to the trigeminal nerve have been reported in breast cancer, prostate cancer and colon cancer. To the best of our knowledge trigeminal neuropath...

Teaching NeuroImages: perineural spread of basal cell carcinoma along the trigeminal nerve.

A 72-year-old man presented with painless left cheek and upper lip paresthesias for 1 year. A basal cell carcinoma had been resected from the left lower lid 2 years before. He had hypesthesia in the left V2-V3 distribution. MRI demonstrated findings suggestive of perineural spread (figures 1 and 2). Local radiation arrested the process. Perineural spread occurs in 2.5%–5% of head and neck cance...
